HealthLinks is a program through Phoenix Day that supports the health and well-being of our community through application assistance in health and nutrition programs for eligible low-income individuals and families as well as linkages to community resources as needed.

Our HealthLinks program has a current opening for a Full Time Spanish/English Bilingual Retention and Outreach Coordinator.

The Coordinator works as part of the HealthLinks team to help individuals and families apply for AHCCCCS and KidsCare government insurance programs, Supplemental Nutrition Assistance Programs, APS and SRP emergency assistance programs and to link members of the community with other health and human services. The Coordinator will also work with our car seat safety program to assist with recruitment, training and the demonstration of proper installation of the car seats.

Schedule: Full Time, 40 hours per week with flexibility to work evenings and weekends as required.

Pay: $13.00-14.00/per hour

Duties and Responsibilities: This position interacts with all levels, both inside and outside the company (e.g. clients, courts, agencies, etc.).  General responsibilities will include, but are not limited to:

  • Assist with outreach goals providing knowledgeable information dissemination and referral service education through the Phoenix metro area.
  • Participate in community events, including health fairs to inform people about AHCCCS, Food Stamps (now called SNAP), community resources and assist with completing applications manually or on the Health-e-Arizona system.
  • Operates lap-top computers using an online enrollment-screening tool, Health-e-Arizona; able to independently trouble shoot basic problems, completes application forms accurately. Communicate on regular basis with the DES/AHCCCS offices and other agencies.
  • Communicates in a positive and courteous manner while interacting with co-workers, staff, parents, and the community to build productive, collaborative relationships to support the achievement of service goals.
  • Provides advocacy support to families to ensure the development of social skills, self-sufficiency and community influence and participation
  • Provides information to families on community resources and coordinates referrals for services as needed.
  • Coordinate activities with the HealthLinks team and submit weekly and monthly activity reports.
  • Clerical tasks (program tracking, filing, faxing, e-mailing and phone responses).
  • Complete National Car Seat Safety Accreditations in order to facilitate and educate community members on the proper installation of cars for children 0-5 years. Participate in creating strategic plan for HealthLinks program. 


  • High school diploma or GED plus related experience is required.
  • Must be Spanish/English bilingual (speak, read and write). Bilingual skills must meet the agency standard and have the ability to communicate effectively in Spanish and the ability to interpret and translate.
  • Have the skills necessary to engage and work with others from diverse ethnic and cultural groups, coupled with maintaining professionalism at all times.
  • Demonstrate and provide strong communications skills both verbally and in writing.
  • Must be committed to teamwork and have the ability to establish and maintain effective working relationships coupled with the ability to work independently or to initiate action.
  • Knowledge of the Kids Care / AHCCCS system and human service programs, a plus. 

Additional Qualifications:  

  • Possess a valid driver’s license with reliable transportation
  • Current auto liability insurance and a clean 39-month motor vehicle record.
  • Have a Class One Fingerprint Clearance Card or be able to pass various background checks in order to obtain one.

Pay Rate: $13-14.00/hour
